When you visit our dental office, we ask that you are transparent with us as we are with you. Our new patient forms are provided below, which can be printed and filled out before your first visit. It is not required that you fill out the forms before attending your appointment, although this will save you and our staff time during your visit.

How do I access and complete the new patient forms?

You can download, print, and fill out all required forms from our Patient Information page before your first visit, which helps speed up check-in on appointment day. 

 If you prefer, you may also arrive 10–15 minutes early to complete the forms in our office.

What should I bring to my first appointment?

Please bring a valid photo ID (driver’s license or government-issued ID), your dental insurance card (if applicable), and a list of any prescription or over-the-counter medications you take. 

If you have recent X-rays or referral forms, feel free to bring or have them forwarded to our office beforehand.

What payment options are available?

We accept cash, personal check, and all major credit cards (Visa, MasterCard, Discover, American Express) both in-office and via our secure online portal. 

 You’ll be asked to sign a Financial Consent Form before treatment, and payment is expected at the time of service unless other arrangements are made.

Do you offer financing or payment plans?

Yes—we partner with CareCredit and other third-party lenders to provide flexible financing options that let you pay over time. 

Please let us know at your consultation if you’d like information or assistance applying; decisions are usually instant.

Will you file my dental insurance claims?

We file all claims as a courtesy and will verify your benefits at your first visit.

You are ultimately responsible for any portion not covered by your insurer; if your insurance hasn’t paid within 45 days, we’ll ask you to remit payment within 15 days.

What if I have questions about my bill or insurance?

Your initial appointment includes a comprehensive oral exam, review of your medical and dental history, any necessary X-rays, and a discussion of your personalized treatment plan.

We’ll work with you to find appointment times that fit your schedule, and we request at least 24-hour notice for changes to minimize late-cancel fees.
